New Patients at Health 4 Life Chiropractic
Welcome to our practice! We’re excited to meet you and help you get the results you want and deserve. Come in, and get comfortable; we promise we won’t keep you waiting long. Relax and have a chat with your neighbor; you’re in a safe space. Please bring your ID and insurance card with you. If you haven’t filled out your forms before coming in, a staff member will make sure you complete all the necessary paperwork before you meet with the doctor. This first visit is about 45 minutes.

When You Come to the Office
After all the paperwork is done, you’ll get a tour of the office with one of our chiropractic assistants, before heading to the new patient exam room. The CA will review your intake forms to get an idea of what your concerns are, and check your vitals. Next is a short introduction video to the practice, before the doctor comes to the room. Your doctor will address any questions or concerns you may have, and whether you wish to proceed with care. If so, he’ll do a physical examination, including posture checks, a full spinal exam, and X-rays.
The Second Visit
This visit is usually about 30 minutes. Before reviewing your X-rays, you’ll watch a short video explaining what information we are looking to obtain from your films, so you know what to expect. Your doctor will put the X-rays up on the screen and walk you through what was found and how it’s affecting your health. You’ll get a brief overview of how we plan to help you and do your first adjustment at this time. We’ll also go over what a typical care plan looks like, so you can choose what level of commitment you’re at and what you hope to get out of chiropractic care. Your doctor will then create your unique care plan.
Customized Payment Options
On the next visit our office team member will go over your plan in detail, along with the cost, insurance copays, deductibles, etc. Our team member will help you find the best payment option. We accept some insurance plans and Medicare, and file all claims for your convenience. Major credit cards are accepted, and flexible payment plans are available.
